What is Advanced Health Care?

  • Advanced heatlh care is much like direct primary care (DPC). It is a health care delivery model that provides an opportunity for patients and providers to enter a relationship that puts the patient first. By providing services for a low, flat monthly fee, patients get same-day or next-day visits without copays. With direct access to the physician, APRN, PA, patients can also contact their doctor by phone, secure text, email, or video chat when care is needed. This practice offers unhurried office visits, access to discounts on medications, labs, pathology, and imaging, all without the need for insurance. Removing poorly performing insurance and delivering affordable care at a great price. This is truly health care.

Is this practice or the DPC movement insurance?

  • No, This practice nor DPC is insurance.  The monthly service fee is your payment for ongoing care services offered by your provider. We strongly recommend that patients do have insurance for care that may be needed beyond what we offer (specialty care, ER or hospitalization, surgery, PT, etc.). Direct primary care and this practice couple nicely with a high deductible or short term accident plan.

I have Medicare can I still become a patient?

  • Absolutely! You will need to sign a one-time waiver declaring that neither you nor our office will directly bill Medicare for our services. We recommend continuing care with your primary care provider (PCP) and allowing us to place the advanced labs testing which are not covered by your insurance based PCP. You may even find that you will save money on your primary care, labs and medications through our discounted pricing. Most importantly we do not provide Medicare services. As we intend to provide an annual routine physical with annual follow up, this is not a covered Medicare service.

What if I need a hospital or am traveling?

  • By providing excellent care with virtually unlimited access to your doctor, we seek to prevent or reduce hospitalizations. Since we no longer provide inpatient hospital care, when hospitalization is necessary we will collaborate with the hospitalist during your hospital stay. We are able, however, to make social visits and act as a liaison and help to make best decisions along with your family members. The goal is to support you and your health care needs.
  • This is one of the benefits of being a patient of A2Z Wellness —because we know you well, we can usually care for you even when you’re away from home. Many illnesses can be diagnosed and treated by communication through phone, secure text, email, or webcam. If necessary and appropriate, we can call prescriptions into the pharmacy nearest you as well as providing paper prescriptions allowing you to travel and process at any pharmacy of your choice.

Are charges tax deductible?

  • We accept HSA payments as an option. Credit, Debit Cards, and ACH are easiest as can be processed online with our Hint intake/billing services.

Will any of my records be shared with insurance companies?

  • No, this is your decision who will see your records. You own your records and will have access to all information through your Spruce Hippa Compliant app Which will work as chronological timeline to your care.

How long is my contract and if I want to discontinue membership?

  • As stated in our agreement you are able to cancel with a one month notice. But if you decide to return you will be asked to pay a new registration fee.

If I enroll in A2Z Wellness when does billing begin?

  • After signing up for service we will charge the registration fee and the first monthly fee which will be billed monthly based on your start date. All contacts are renewed monthly based on sign up dates.

Nurse Practitioner Justin AGACNP-BC

Justin Chamberland is a board-certified acute care nurse practitioner with over 20 years in the healthcare industry, focusing on the management of chronic disease with a functional medicine approach, which involves alternative therapies to increase quality of life. He  studied as a mechanical engineer undergraduate at the University of New Hampshire and Vermont, later pursuing nursing at the University of Central Florida and the University of Purdue Global.

NP Justin discovered his passion for healthcare while living overseas in Costa Rica and Nicaragua. During that time living internationally he was able to learn the importance of personalized healthcare focused on listening to patients.  Justin wants to engage clients in health promotion activities to increase mental well being and stress reduction. 

Justin and his wife, Janeth, live on Brevard County’s beautiful coastline where they have raised their son and two daughters.  Justin has continued as a nationally certified volunteer soccer coach as well as mentoring youth through action sports including the establishment of a global non-profit, “Ride Nature.”  In their spare time they enjoy hiking Florida’s trails and visiting Florida’s wonderful natural springs.
